We strive to provide patient focused reviews that are relevant to the individual patient, rather than providing reams of generalised information.
In Recognition of the growing need for pharmacists to provide quality medication review services, as part of a wider healthcare team, we are pleased to introduce our Medication Review Team.
Our team has many years of experience in providing quality clinical pharmacy services to hospitals, aged care homes and patients in the community.
Our reports are concise and relevant to the individual patient, and take into consideration the most current disease state management guidelines.
All referrals are responded to within 2 business days;
Urgent referrals will be followed up on the day of receipt;
Urgent issues are communicated to the GP immediately;
We understand that you may be busy with work or family commitments so appointments can be made to fit your schedule. Regular week day or after hours and weekend appointments are available.
Medication Management Outside the Surgery
Various Australian research projects have established that medication mismanagement and adverse drug reactions result in about 140,000 hospital admissions per year.
As our population ages and disease management guidelines are refined, the prevalence of poly-pharmacy will increase.
Home visits by pharmacists can give valuable insight into problems experienced by patients, which may be impacting on their ability to cope with their medication regime.
These problems may not necessarily be apparent when a patient is at the doctor’s surgery e.g. large quantities of expired medicines held at home, administration technique problems (puffers, injections, nebulisers), incorrect storage conditions etc.
By performing a HMR, undetected medication issues can be identified and problems may be solved before they become serious and result in a hospital admission.
Following a HMR, the patient is more likely to take their medications as prescribed. They will also have a better understanding of the reasons for taking their medicines.
According to the available evidence this should result in an improved patient outcome.
Which patients would benefit from a HMR?
The following is a list of reasons to use The Wesley Pharmacy HMR service
· Currently taking FIVE or more medications
· Currently taking more than TWELVE doses of medication per day
· Significant changes made to medication regimen in the last THREE months
· Taking medications with a narrow therapeutic index or requiring therapeutic
monitoring
· Symptoms suggestive of an adverse drug reaction
· Sub-therapeutic response to treatment
· Suspected non-compliance or inability to manage therapeutic devices
· Difficulty managing medication due to literacy and language difficulties, dexterity
problems,impaired sight and/or confusion or other cognitive difficulties
· Attending a number of different doctors, both GPs and specialists
· Recently discharged from a facility/hospital in the last TWO weeks
· Due to be admitted to a facility/hospital in the next FOUR weeks for major surgery
· The GP believes that the patient will benefit from a complete review of their
medicines.
